You may download and print these documents as necessary.  

Please remember that you may only register your child if you have been called and a spot is available.  Additional paperwork for registration and enrollment will be required and those are available for pick up when you enroll your child. 

Registration Forms and Enrollment forms will be filled out on-line via Brightwheel once your child has a confirmed space at our school.  These are additional documents that will be needed. 

• Immunization Record Form (*even if your child's physician provides a separate shot record document, THIS form MUST be signed as per licensing requirements)
• Permission to Administer Medication Form
